“Total Access Medical” refers to Total Access Medical LLC, a Delaware limited liability company. Total Access Medical is the company that has designed the Total Access Medical Program, and will administer certain of the non-clinical aspects of the Total Access Medical Program. Total Access Medical is not a provider of medical or other healthcare services, and affiliated physicians are not employees of Total Access Medical. The Total Access Medical Program is not an insurance plan or arrangement and patients must be certain to have health insurance in place at all times. So long as an individual participates in the Total Access Medical Program, TAM affiliated physicians will have sole authority over and responsibility for the provision of medical services to enrolled patients. Total Access Medical will, however, be available to provide the non-clinical services in an effort to make the experience of being cared for by a TAM affiliated physician as convenient and as effective as possible. Neither Total Access Medical nor the physicians are responsible for the obligations of the other.
